credit cards, credit unions, mortgagees, auto loan, support payment, etc.

Names, addresses and
telephone numbers of nearest living relative

Bankruptcy (if applicable)
copy of Discharge, Schedule of debts, letter of explanation of slow pay
items, list of accounts that were reaffirmed

Other income including:

Pension, disability,
social security, education income (current letters from source stating
amount received)

Child support (copy of
divorce decree and supporting evidence that amount has been received for
the last 2 years

Rental income (copy of
leases, statement showing rent collected monthly, maintenance and
utilities for each property, annual taxes, last 2 years Federal Returns,
including schedule E)

last 3 years tax returns including all schedules, profit/loss and
